What LL-37 is

LL-37 is a 37-amino-acid antimicrobial peptide cleaved from the precursor protein hCAP18 — the single cathelicidin produced by the human body. It is expressed by neutrophils, epithelial cells, and skin as a first-line innate-immune defense, and it does more than one job:

  • Direct antimicrobial: disrupts microbial membranes and biofilms across bacteria, fungi, and viruses
  • Wound repair: studied for angiogenesis, cell migration, and re-epithelialization
  • Immunomodulation: recruits immune cells and tunes the inflammatory response

Most of this evidence is preclinical. Human therapeutic data is limited, and LL-37 is best understood today as a research compound rather than an established treatment.

Safety

The dual-edged biology

LL-37 is genuinely double-sided. The same peptide that is protective in one setting can be pro-inflammatory in another:

  • Elevated LL-37 has been implicated in inflammatory skin conditions such as psoriasis and rosacea
  • It can act as a self-antigen and amplify inflammation in some autoimmune contexts
  • Effects depend heavily on concentration, tissue, and the surrounding immune environment

That complexity is why candidacy, route, and dosing would need careful physician oversight, and why LL-37 is not a routine therapy. Discuss the risk/benefit profile with a licensed physician.
