SF Peptides (“we,” “us”) is a peptide and skincare brand powered by CompoundMD. Where we offer prescription products, care is provided by licensed clinicians through telehealth — the delivery of healthcare using secure technology such as video, audio, messaging, and electronic forms instead of an in-person visit. This consent explains how telehealth works and what you agree to when you use it. By requesting a consultation, you acknowledge that you have read and agree to it. Our GHK-Cu serum is a non-prescription topical cosmetic and does not require a telehealth consult or this consent.
Possible Benefits of Telehealth
Telehealth lets you connect with a licensed provider at a time and place that is convenient for you, without traveling to an office. It can improve access to care and make follow-up easier.
Possible Risks & Limitations
Telehealth has limitations you should understand before consenting:
- Your provider cannot perform a hands-on physical examination, which in some cases is important to a diagnosis.
- The provider relies on the information you share; incomplete or inaccurate information can affect the assessment and may lead to an incorrect or delayed diagnosis.
- Technical problems (connection, audio, or video issues) may interrupt or delay a visit.
- Telehealth is not appropriate for every condition, and is never appropriate for emergencies.
- As with any electronic transmission of health information, there is a small risk that security measures could fail or that data could be accessed by unauthorized persons.
Your Consent to Telehealth
Provider qualifications & no guarantee
Your care may be provided by a licensed physician, nurse practitioner, or physician assistant, depending on your state and your needs. A prescription is issued only if a licensed provider determines it is clinically appropriate. No specific result or treatment is guaranteed, and a provider may decline to prescribe.
Your understanding & responsibilities
You agree to provide complete and accurate medical information, and you understand that your provider may determine that telehealth is not appropriate for you and that in-person care is needed instead. Submitting an intake or quiz does not by itself create a provider-patient relationship — that relationship begins only upon completion of a telehealth consultation with a licensed provider.
Consent & withdrawal
Your participation is voluntary, and you may withdraw your consent to telehealth at any time by contacting us in writing. Withdrawal applies going forward and does not affect care already provided.
Electronic communication & privacy
Your personal and medical information will be transmitted and stored electronically and maintained by the licensed telehealth provider on a HIPAA-compliant platform, used to provide your care. Website and marketing data are handled under our Privacy Policy, and health data covered by applicable consumer-health-data laws is handled under our Consumer Health Data Privacy Policy.
Costs
You are responsible for the costs of the products and services you purchase, as disclosed at checkout. Our services are cash-pay; we do not bill insurance or seek Medicare/Medicaid reimbursement on your behalf.
Not for Emergencies
Telehealth is not for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room.
