What tesamorelin is

Tesamorelin is a synthetic analogue of growth hormone-releasing hormone (GHRH). It binds to GHRH receptors in the anterior pituitary, stimulating somatotroph cells to produce and release growth hormone in natural pulses. GH then stimulates the liver to produce IGF-1.

The GH/IGF-1 axis promotes lipolysis (fat breakdown), particularly in visceral adipose tissue, while preserving lean muscle mass. Because GH is released in response to tesamorelin rather than injected directly, the pituitary's negative feedback remains intact — GH cannot over-accumulate, which reduces acromegaly risk.

Who it's for

Not all fat is equal. Subcutaneous fat under the skin is largely cosmetic. Visceral fat — the deep abdominal fat packed around the liver, pancreas, and intestines — is metabolically active. It secretes inflammatory cytokines, drives insulin resistance, and is strongly associated with cardiovascular disease, type 2 diabetes, and metabolic syndrome.

Standard weight-loss diets reduce visceral and subcutaneous fat proportionally. Tesamorelin is one of the few interventions shown to preferentially reduce visceral fat — making it relevant for patients with central obesity even at normal BMI. Safety

Side effects & contraindications

Tesamorelin is contraindicated in active malignancy, disruption of the hypothalamic-pituitary axis (from tumor, surgery, or radiation), and pregnancy. Patients with diabetes should be monitored closely, as GH can impair insulin sensitivity.
