GLP-1 Weight Loss
Tirzepatide
A dual GLP-1/GIP receptor agonist (Mounjaro/Zepbound) that produced the highest weight loss numbers ever recorded in a pharmaceutical trial. Compounded for California patients via telehealth — physician consultation, prescription, and pharmacy delivery.

Why patients choose it
Why patients choose tirzepatide
Dual-receptor mechanism
Activates both GLP-1 and GIP receptors at once — a one-two effect on appetite and insulin sensitivity that semaglutide's single pathway can't match.
Strongest trial results
SURMOUNT-1 showed up to 20.9% body weight reduction at the 15mg dose over 72 weeks — exceeding semaglutide in head-to-head data.
Once-weekly, at home
A single subcutaneous injection per week, titrated slowly to minimize GI side effects. No clinic visit required for California patients.
Overview
What tirzepatide is
Tirzepatide is a once-weekly injectable peptide that activates two receptors simultaneously: GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). This dual mechanism is what distinguishes it from semaglutide, which targets GLP-1 alone.
It was FDA-approved under the brand name Mounjaro for type 2 diabetes in 2022, and as Zepbound for chronic weight management in 2023. Compounded versions are available from California-licensed pharmacies when brand product is on the FDA shortage list.
Candidacy
Who it's for
Tirzepatide is most commonly pursued by adults seeking meaningful weight loss or better metabolic control — particularly those who haven't reached their goals through diet and exercise alone, or who responded only partially to a single-pathway GLP-1.
California patients who ask about tirzepatide typically describe some combination of:
- A BMI in the overweight or obese range with weight that won't shift
- Type 2 diabetes or insulin resistance alongside weight concerns
- Strong appetite or food noise that undermines dietary changes
- A plateau or insufficient response on semaglutide
What to expect
How it unfolds, week by week
01
Weeks 1–4
Start at 2.5mg once weekly. This is the GI-adjustment phase — appetite often drops noticeably while your body adapts to the medication.
02
Weeks 5–12
Titrate to 5mg, then 7.5mg if needed. Early weight loss typically becomes apparent in this window, usually within the first 4–8 weeks.
03
Weeks 13+
Titrate to 10mg, 12.5mg, or 15mg based on tolerance and response. Slower escalation reduces nausea significantly — do not rush dose increases.
Tirzepatide vs. Semaglutide
| Tirzepatide | Semaglutide | |
|---|---|---|
| Receptor targets | GLP-1 + GIP | GLP-1 only |
| Max trial weight loss | ~21% | ~15% |
| Dosing frequency | Once weekly | Once weekly |
| FDA approval | Mounjaro / Zepbound | Ozempic / Wegovy |
| Compounded cost (CA) | $200–400/mo | $150–350/mo |
Results
Clinical results
The SURMOUNT-1 trial enrolled 2,539 adults with obesity (no diabetes). At 72 weeks, weight loss scaled with dose: roughly 15% at 5mg weekly, 19.5% at 10mg weekly, and 20.9% at the 15mg weekly dose — compared with 2.4% on placebo.
Source: Jastreboff et al., NEJM 2022. Results in patients without diabetes on 15mg/week at 72 weeks. Individual results vary, and patients who pair tirzepatide with dietary changes and exercise tend to see better outcomes.
Safety
Side effects & contraindications
Gastrointestinal side effects are the most common, especially during dose escalation:
- Nausea and vomiting (usually diminishes over time)
- Diarrhea or constipation
- Reduced appetite and early satiety
Starting low (2.5mg weekly) and titrating slowly reduces GI burden. Rare but serious risks include pancreatitis, gallbladder disease, and — in animal studies — thyroid C-cell tumors (not confirmed in humans at therapeutic doses). Your prescribing physician will screen for relevant contraindications.

Cost in California
Compounded tirzepatide runs $200–400/month from a California-licensed pharmacy, depending on dose. Brand Mounjaro/Zepbound, if covered by insurance, can reduce out-of-pocket costs substantially. Consultation fees are $99–199. See our full cost guide.
